SUMMARY: problems with Sun X6758a LVD controller

The reply from Jay (below) hit the nail on the head - I was missing
SUNWqus.  Since the card was represented as being supported
by S8 2/02 (impossible, since the card wasn't announced when
S8 2/02 shipped), I plan on returning it.
