A massive heaping of thanks to Roy Rapoport for all of this help and 
guidance as well as these kind folks for their response and narrowing down 
my hypothesis (as well as correlating Roy's input):

The evil patch cluster clobbered inetd.conf - making the issue a bit more 
exciting was the fact this is a production box and with a clobbered 
inetd.conf file I couldn't use my tape-backup environment to restore - so 
with a good amount of guidance, luck, Google and Manuals the problem was 
solved.

Pertinent:inetd.conf entry:

imap stream tcp nowait root /usr/local/sbin/imapd imapd

[the pathname has to be specific to your imapd binary]

Veritas Netbackup entry:

bpcd stream tcp nowait root /usr/openv/netbackup/bin/bpcd bpcd
