The problem was a non-bootable Ultra-E4000 after upgrading the
obp from 3.2.10 to 3.2.12. Attempting to boot from any scsi
devices resulted in an error message "Fast Data Access MMU
Miss".
Well, it seems from pulling the box apart that the culprit was
an sbus hba with a tachion-2 chip which was causing the
io-board bus to hang. After removing this all was fine. Also,
the newer tachion-3 chip works ok -- so far...
