These files exist?
% ls -l /kernel/drv/logindmux*
-rwxr-xr-x 1 root sys 13288 Apr 11 1995 /kernel/drv/logindmux
-rwxr-xr-x 1 root sys 150 Oct 8 1994 /kernel/drv/logindmux.conf
ls -l /dev/logindmux
crw------- 1 root sys 11,117 Jun 1 11:49 /dev/logindmux
This was enough to have the necessary input for mknod.
My /etc/name_to_major:
logindmux 118
mknod logindmux c 11 118
crw------- 1 sys 11,118 Oct 6 13:52 /devices/pseudo/clone@0:logindmux
crw------- 1 other 11,118 Oct 9 08:19 /dev/logindmux
