I was able to resolve my problem last night
with the following steps.
1) attach cdrom
2) load mini unix
3) mount /dev/rid000a a
4) fsck /dev/rid000a
5) reboot
The blower assembly in the 4/390 had failed
and probably caused or contributed to the problem.
The card-cage was VERY warm to the touch, I replaced
the blower and all is well today. Below is my original
problem.

Check your SCSI chain. Remove components, swap cables etc.
