I recently asked for the format.dat entry for the Fujitsu M2654SA SCSI
hard disk.

The format.dat entry I settled on and works for me is:

disk_type = "FUJITSU M2654SA" \
        : ctrl = SCSI : fmt_time = 4 \
        : ncyl = 2170 : acyl = 2 : pcyl = 2197 : nhead = 21 : nsect = 88 \
        : rpm = 5400
